Why a single ECG isn’t always enough

An electrocardiogram is one of the most valuable tools in cardiology — it records the electrical impulses that coordinate each heartbeat, showing rhythm, rate, and conduction in real time. But the standard resting ECG only captures what’s happening in that brief moment, and many of the most clinically important disturbances are intermittent. Atrial fibrillation may come and go unpredictably. Premature beats may cluster at night or during stress. Exercise-induced arrhythmias won’t show up while you’re sitting still in an exam room.

FOUNDATION

Resting 12-Lead ECG

Immediate information about your heart’s rhythm, rate, and electrical conduction. We use it as a baseline and to evaluate for structural changes, conduction delays, or acute findings — an essential starting point, though limited in duration.

24–48 HOURS

Holter Monitor

Records every heartbeat continuously over one to two days during your normal activities — sleep, exercise, meals, stress. Valuable for detecting intermittent arrhythmias, assessing premature beats and heart rate variability, and correlating rhythm changes with your symptoms.

7–30 DAYS

Extended Event Monitoring

For symptoms that occur infrequently, a lightweight wearable tracks your rhythm over one to four weeks. Press a button when you feel symptoms and the device correlates your experience with the objective recording — a much higher chance of capturing the responsible disturbance.

UNDER EXERTION

Exercise Stress ECG

A treadmill test observes how your rhythm, rate, and blood pressure respond to exertion — useful for exercise-induced arrhythmias and assessing whether coronary blockages may impede blood flow. Paired with a peak-exercise echocardiogram, it reveals even more about heart-muscle function.

01 I had a normal ECG at my doctor’s office. Why might I still need monitoring? +
A standard resting ECG captures only about 10 seconds of your heart’s electrical activity. Many rhythm disturbances are intermittent — they come and go — and may not be present during that brief recording. If you’re experiencing symptoms like palpitations, dizziness, or unexplained fatigue, extended monitoring over days or weeks dramatically increases the chance of capturing the abnormality and giving you a definitive answer.
02 What does wearing a monitor feel like? Will it interfere with my daily life? +
Modern cardiac monitors are small, lightweight, and designed to be worn during your normal activities. Most patients forget they’re wearing them after the first few hours. The goal is to capture your heart’s behavior during real life — including sleep, exercise, work, and stress — so we encourage you to go about your day as normally as possible while wearing the device.
03 Can stress or anxiety cause the palpitations I’m feeling? +
Stress and anxiety can absolutely contribute to palpitations and rhythm changes — but that doesn’t mean they should be dismissed without investigation. Chronic stress elevates cortisol, disrupts electrolyte balance, and creates an inflammatory environment that can trigger or worsen real arrhythmias. We confirm exactly what’s happening electrically with monitoring, then investigate the underlying drivers — including stress biology — so we treat the root cause rather than just labeling symptoms as “anxiety.”
04 What happens if the monitor detects something abnormal? +
If we identify a rhythm disturbance or conduction abnormality, we’ll review the findings with you in detail and explain what they mean for your health. From there we build a personalized plan that may include further diagnostic testing, lifestyle and metabolic interventions to address root-cause contributors, and in some cases referral for specialized procedures or medication management. 05 Do I need a referral for ECG monitoring? +

06 Is ECG monitoring available through telehealth? +
The consultation, ordering, and results review can all be conducted virtually. For certain types of monitoring, a wearable device will be shipped to you or coordinated through a local facility. We’ll ensure the process is as convenient as possible regardless of your location.
